We're happy you're interested in becoming part of our community. Please submit the New Client Request Form below and we will send you a link to complete our intake paperwork.  Once your account is set up, we will contact you for scheduling.  Feel free to request a specific provider.  

Not sure which therapist is the right choice?  Our Clinical Intake Coordinator has 20-years of experience as a therapist and works closely with all of our providers.  After reviewing your paperwork, the coordinator will reach out to discuss therapist options and availability.
